Program Information
What are the start and end dates for the Program?
The Program begins on January 1, 2021 at 9:00:00 AM CT and ends on December 31, 2025 at 11:59:59 PM CT. All purchases made during this period are eligible, subject to the Terms and Conditions. The Sponsor reserves the right to make any modifications to, or terminate the program at any time.
How many days after purchase do I have to file a claim for my reward?
Purchase claims must be submitted within 60 days of the purchase date on your receipt.
Who is eligible for the See Rewards™ Program - LensCrafters?
This Program is open to all residents of the 50 United States, the District of Columbia, and Puerto Rico who are at least 18 years of age at the time of registration (19 in AL and NE), and have a valid email address.
How do I become a member of the See Rewards™ Program - LensCrafters?
Become a member of the See Rewards™ Program - LensCrafters by visiting the Program Website at www.SeeRewards.com/LC and registering. Registration includes providing certain information about yourself. You will be requested to provide your full name, zip code, date of birth, email address and password, and confirm that you accept the Official Terms and Conditions.
Once you are registered, you can sign into your account at the Website with your email address and password.
How do you use my information? Do you share my information with anyone?

We will use your information to administer this Program and as indicated in our Privacy Policy. We will not share your information with other organizations except as set forth in our Privacy Policy.
Are there any restrictions to this Program?
Participation in the Program is only available to registered members of the See Rewards™ Program - LensCrafters who are eligible to participate. Please note that you must be a legal resident of the 50 United States, the District of Columbia, or Puerto Rico, and 18 years of age or older (19 in AL and NE), to be eligible to join the See Rewards™ Program - LensCrafters.
Some limitations apply to all offers: For example, there is a limit of one (1) submission per person per 12-month period, a limit of five (5) submissions per address per 12-month period, and claims must be submitted within 60 days of the purchase.

Entering a Claim
How do I submit a claim?
Your claim must be submitted on the Program Website using the Contact Lens Offer Submission Form provided on the website.
Your completed Claim Form, to be eligible, must be accompanied by:

a legible image of your receipt from your eye care professional (credit card receipts and packing slips will not be accepted), showing:
- proof of purchase of LensCrafters One Day Daily Disposable Lenses (name and quantity)
- Purchase date within the time period indicated on the Offer Form or other offer information materials (purchase date will be determined by the date on the receipt submitted);

a legible image of all eligible boxes and corresponding UPC(s) from all eligible product packaging.

Where can I find UPCs to submit?
UPC Codes from participating LensCrafters products must be submitted as part of a claim on the website using our Contact Lens Offer Submission Form.
IMPORTANT: Before you attempt to submit a claim, please visit your eye care professional at a participating LensCrafters location to obtain an Offer Code (and/or an Offer Form or other offer information materials, which will give you access to the offer and describe the participating products, as well as how to submit your claim).
What is the difference between a "New Wearer" and an "Existing Wearer"?
Some Point-earning offers may indicate that they are only available to "New Wearers" of a participating product. A "New Wearer" is someone who has not previously purchased the respective LensCrafters One Day Daily Disposable lens (including modality, e.g. SVS, Astigmatism or Presbyopia) subject to the claim being submitted and who has not submitted a claim for rewards for such purchase to See Rewards within the two (2) year period preceding the current purchase of qualifying lenses subject to the current claim for rewards and an "Existing Wearer" is anyone who does not qualify as a New Wearer.
My Contact Lens Purchase Claim was approved, now what?
Congratulations! You are one step closer to receiving your See Rewards™ Visa® Reward Card. Once approved, you will receive an email with a confirmation that your See Rewards™ Reward Card has been ordered and you will receive it in the mail within 6-8 weeks.
My Contact Lens Purchase Claim was rejected, now what?
If your purchase claim for contact lenses was rejected, our moderation team will provide you with one or more reasons why it was rejected. The most common errors we see are:
- Not including a receipt that clearly shows the LensCrafters product purchased as well as the quantity
- Providing a credit card receipt or packing slip instead of a receipt from your LensCrafters eye care professional
Within your rejection email you will receive a link back to your claim. This link will allow you edit any missing or incorrect information and resubmit for approval.

How do I check the status of my claim?
Login to your See Rewards™ Program - LensCrafters account and click on MY ACCOUNT to review your account activity. If your claim has been rejected, you can edit your claim from the MY ACTIVITY page as well.
See Rewards™ Visa® Reward Card
Visa Reward Card is issued by Pathward®, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Cards can be used everywhere Visa debit cards are accepted in the U.S. and District of Columbia. NO CASH OR ATM ACCESS. Terms and Conditions apply. See Cardholder Agreement for details.

When will I receive my See Rewards™ Visa® Reward Card?
Visa® Virtual Account
In approximately 48-72 hours of your contact lense purchase claim rewards approval, your Visa® Virtual Account order will be sent to our Visa® issuer, which will trigger an email from our Visa® issuer with a link and instructions on how to redeem the prize. Visa® Virtual Account cards are emailed within 5-10 business days.
Visa® Virtual Account is issued by Pathward®, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Terms and conditions apply. Virtual Account can be used online, via phone, or in store with a digital wallet everywhere Visa debit cards are accepted in the U.S. and District of Columbia. Visa® Reward Cards
In approximately 48-72 hours of your contact lens purchase claim rewards approval, your Visa® Reward Card order will be sent to our Visa issuer. Your Visa® Reward Card will be mailed to the address that was in your profile when your reward was confirmed. Visa® Reward Cards are usually delivered within 4 weeks to 6 weeks.

How long do I have to use my See Rewards™ Visa® Virtual Account and Reward Card?
Your card is valid for a period of 6 months from the date of issuance.
